The device is the Realtek RTL8821AE 802.11ac PCIe Wireless Network Adapter and the driver is the rtl8821ae.
It also shows (by the tray icon) that it is still connected but the wifi / internet doesn't work at all (webpages hang up etc.) This can also often happen even from a fresh boot, it says connection established yet the internet doesn't work at all. Issue - It will constantly but intermittently disconnect, seemingly without cause.
